Southern Plains Inter-Tribal Epidemiology Center:
Tribal Community Health Profiles Project

This project aims to provide health specific data for tribal communities with the Area to guide tribal leaders in developing a community health plan. Morbidity data are provided by the Area from reported hospital and clinic data. Community specific key social determinants of health status and behavioral indicators in existing data sets are collected and tabulated.

Choctaw Nation of Oklahoma and Citizen Potawatomi Nation are the first tribes scheduled for CHP completion. CHPs are planned for development for the remaining 40 tribes in the Oklahoma City Area.
